I have created a concussion survey for all of those out there willing to help out. This survey is meant for data collection, completely non-identifiable and non-scientific. The questions include; sex, age, grade, sport, game/practice, who evaluated, how did they evaluate, hospital trip, and return to play. PLEASE PROVIDE INJURY INFORMATION FOR THE 2011-2012 SCHOLASTIC YEAR FOR HIGH SCHOOL STUDENT/ATHLETES.
The survey is 10 questions in length and should take you no time, I would appreciate all the responses possible (please re-visit for each of your concussions).
